What kind of healthcare plans does High Performance offer?

High Performance offers several options for 1099's, Independent Contractors, Entrepreneurs and Small Businesses. We have a self-funded employer pool with a fully funded backstop within a group captive or associations. We also offer private healthcare plans.

Is this an ACA Marketplace plan?

No. The private plans are bona fide group health insurance held in a captive, an employer-sponsored group product governed by ERISA. It is not an ACA Marketplace (individual exchange) plan.

What is the relationship with Cigna?

Our partner Ameristead has held a direct ASO contract with Cigna since November 2025 (live December 2025). It is the only pool in the country with a direct ASO contract with Cigna, which means Cigna itself can verify member benefits, and all claims flow to Cigna with Marpai adjudicating them for Cigna. It is not a test pilot. The captive that houses the program has been in existence for roughly 26 to 27 years.

When do plans renew?

Renewal is every January 1, and the open enrollment window runs November 1 to December 5 each year. That is when members can make plan changes. Mid-year changes require a qualifying life event (QLE), just like any other group plan.

Which states is the Cigna PPO available in?

All 50 states, with one caveat: Hawaii has its own state rules and extra approval steps, so expect some friction there. Washington, Oregon, New Jersey and the other traditionally tricky states are fine.

Who can enroll?

Because it is bona fide group coverage, each primary member must have an EIN, even if it is just one employee.

Which Cigna network is it?

Cigna’s large commercial PPO, the OAP (Open Access Plus) network. When members visit a provider, they pull up in the Cigna system for benefit verification like any commercial Cigna member.

Which Aetna network is it?

It is the Aetna Signature Administrators PPO network.

How do members access telemedicine?

Teledoctor visits are $0 on every plan. Members download the Opyn app, or log in to the online member portal from a browser.
